Of all the of family documention the "TESTIMONIUM ORTUS ET BAPTISMI" for Antonio Berezik has proven to be the most informitive piece of paper and the greatest link to the Old World.

Within this paper we know exactly what town the Berezik family lived during the year 1888, what church they belonged to and who certian family members consisted of.

The (Diaecesis) = Dioceses was PREMISLIENSIS, the (Districtus)= District and the (Decanatus) = County were both LISCENSIS, and the name of the town and (PAROCHIA) = Parish was at that time LISKO. With the stamp still attached to the orginal document we see that the province was KRONE.

Notice how the dates are written out before the actual number
ie: 1. vigesima = 20
2. octingentesimo = 18
3. oxagesimo = 80
4. octavo = 8

When I visit the town of present day {Lisko} Lesko in the year 2000, I will expect to find my different things. The name of the town is now recongnized as LESKO. I hope to find this document listed in the church records in Volume 5 on page 73 line 17.

Rozalia or perhaps Antonio (Tony) requested this duplicated copy from THE CHURCH OF ARCHBISHOP of ST. NICHOLAS in the town of LISKO back in November 11, 1907. As we can see Antonio was born on April 20, 1888, and both baptised and confirmation on May 6, 1888 which is common in the Greek Catholic Church. Antonio's godfather was Martin Nycz, most likely Rozalia's brother, and his Godmonther was Maria Rudiak both from Lisko.

Antonio's paternal grandparents were Antonii Berezik and Catherine Grzybowski of Lisko.
Antonio's maternal grandparents were Francisci Nycz and Anna Godlowski also of Lisko.

The Name of the preist who baptised Antonio was The Reverend Domino (Domminic) Gregorio (Gregory) Lekieta greek catholic parochia Liscensi.

The priest who duplicated the Baptismal Certificated on November 11, 1907 was Father J BIRECKY Greek Catholic parochia in Lisko.
