She was born 24 November 1842 in Union Co, OH, and died 15 October 1920 in What Cheer, Keokuk Co, IA.

Declaration for Original Pension of an Invalid

State of Illinois
County of Winnebago
      On this 24th day of January, A. D., 1872, personally appeared before me, Clerk of the County Court, a duly authorized officer of a Court of Record in and for the County and State aforesaid, Daniel Hubbard who, being duly sworn according to law, declares a that he is the identical Daniel Hubbard who enlisted under the name of Daniel Hubbard, in the service of the United States, at Pecatonica, Illinois, on the 1st day of August, in the year 1861, as a private in Company "G", in the 44th Regiment Ill. Infty Volunteers, and transferred V. R. C. [Volunteer Reserve Corps] Aug 1st 1863, in the war of 1861, and was honorably discharged on or about the 28th day of December, in the year 1863, and that he has not been in either the military or naval service of the United States since the 28th day of December, 1863 nor has he enlisted, done service, or received pay in any branch of the same since that date; that his personal description is as follows: Age 40 years; height About 5 ft 6 in; complexion, dark; hair dark; eyes. light; that while in the service aforesaid, and in the line of his duty, received the following wound or disability, and that he was In General field hospital Murphysboro [Murfreesboro] Tenn, then Nashville Tenn then at hospital at Jeffersonville Ind. and afterward taken to Hospital No.1 at Elmira N. Y. That while in the line of duty with his said Co at the battle of Stone River Dec 31st 1862, he was wounded by a fragment of a shell striking him in the back on account of which he was taken prisoner by the rebels and conveyed to Libby Prison. Afterwards paroled at Annapolis M. D. and exchanged at St. Louis then conveyed to the hospital as above. That by reason of said wound he (is - crossed out) has been troubled with weakness in the back and bloody piles and his present physical condition troubled with weakness in the back and bloody-piles Cannot do over one quarters of a days work That since leaving the service said Applicant has resided at Pecatonica County of Winnebago and State of Illinois, and his occupation has been Blacksmith.
      He makes this declaration for the purpose of being placed on the Invalid Pension Roll of the United States, by reason of the disability above stated; and hereby constitutes and appoints W. Scott Agney of Freeport, County of Stephenson State of Illinois; his attorney to prosecute this claim and procure a Pension Certificate.
      That he has never received or applied for Pension; that his residence is at No.......in.......street, in the Town of Seward County of Winnebago, State of Illinois, and that his post-office address is Pecatonica Winnebago Co. Illinois

Tiett a Notary Public within and for the aforementioned county and State Sarah J Hubbard widow of Daniel Hubbard of co ((G) 44 regt Ill Vols who after being duly Sworn by me according to law declares that She has no medical evidence that She knows of now to offer of the reason that Dr Newell of Pecatonica Ill was their family physician from the time her Husband above mentioned was discharged untill he died and treated him all through his sickness, the above mentioned Dr Newell now dead debars her from giving medical evidence in support of her claim for pension No. 172132 and that her Post Office address is Morrow Washington County Kansas
