Notes for FORREST E. LADD, SR.:
At the time of his death he was living as a semi-invalid with emphysema on the farm of Nila and Loyd Haskin southwest of Cushing, Oklahoma. In his last illness he was taken by ambulance to University Hospital in Oklahoma City for treatment. He died there the night he was admitted.
