Notes for Avis Eudoria Collins: The death certificate for Avis contains conflicting information. On the certificate, her birth date is given as April 8, 1870 and her date of death is given as July 27, 1923. However, the certificate gives her age at time of death as 50 years, three month, and 19 days. Calculating her birth date from the date of death and age at time of death gives her birthday as April 8, 1873. The cause of death is listed as otitus media which is an infection of the ear. In modern day medicine, the infection is generally successfully treated. The attending physician was Dr. Sneary.
According to her obituary on page 2 of the July 28, 1923 issue of The Daily Examiner published in Bellefontaine, Ohio, Avis' funeral services were held on Monday, July 30, 1923 at the family home with Rev. Mr. Culler of Springfield in charge. I have the agreement between the New Salem church and Frank Clingerman for the purchase of the burial site in the New Salem Cemetery in my files.
