John Rex, a son of John and Sybilla(Sebella) Bastion Rex, was born 11 November 1771 in Germantown, PA. His father owned property in Montgomery and Philadelphia Counties in Pennsylvania. Though it is not known why he departed Pennsylvania, he apparently had arrived in Raleigh, NC, by July 1793, when he purchased half an acre of land from Joel Lane. He died in Raleigh, North Carolina, 29 January 1839. He was buried in the Old City Cemetery after a service at Christ Episcopal Church. He never married.

John was, like his older brother, a tanner, and also a planter. In his early twenties he moved to Raleigh and remained there for the rest of his life. He eventually bought slaves and owned eighteen at the time of his death. John Rex is most well known for the terms of his will and its results. His property in Pennsylvania, the Broad Axe Tavern, was left to a kinsman and namesake living in Pennsylvania. More interestingly, his will directed his slaves to be freed and sent to Liberia under the auspices of the American Colonization Society, with certain assets to be sold to be provided for their transportation and settlement. Sixteen of the eighteen slaves left 4 August 1839 aboard the ship Saluda for Liberia, among them Abraham Rex, age 27, tanner and shoemaker, later identified as John's son by the slave Malinda. Abraham returned from Africa by the summer of 1840, but did not remain in North Carolina because of legal provisions regulating manumitted slaves. He met some people headed for Buffalo, New York, who "induced him to go with them by their fair tales." In 1843 he considered returning to Africa but by 1850 he had removed from Buffalo and his ultimate fate is unknown. The remainder of his estate, including two lots southwest of Raleigh, was to be used for the erection of a hospital "to provide a comfortable retreat for the sick and afflicted poor belonging to the city of Raleigh."

Duncan Cameron and George W. Mordecai served as the executors of the estate. Before its terms were carried out, many lawsuits, including two before the North Carolina Supreme Court, were heard. The American Colonization Society battled the supreme court to obtain the funds legally entitled to the freed slaves. Nine of the original 18 who settled in Liberia eventually collected these funds in 1856 and 1857. Establishment of the hospital was also delayed many years, in part because much of the investment was lost during the outbreak of the Civil War. Rex Hospital was eventually established in 1894 in the old Charles Manly house on South Street in Raleigh.

No likenesses of John Rex are known to exist, although his acquaintance David L. Swain described to him as bearing a "striking resemblance to John Quincy Adams." In character, Swain described John Rex as "a grave, sedate, quiet, retiring, modest man."
